Palm Beach Neurological Center Patient Resources

 

The Palm Beach Neurological Center is a private practice under the direction of Michael M. Tuchman MD, FAAN Board Certified Neurologist, dedicated to the modern evaluation and treatment of neurological disorders.  We emphasize a team approach to patient care with an active role for the patient and the family. Appointments

 

We make every effort to see all patients at their appointed time and request that you extend the same courtesy to us. On occasion emergencies do arise and whenever possible, you will be informed of any delay. We request that you notify our office immediately if you will be late for your scheduled appointment. This allows us to utilize the time to attend to other patients.

 

On your first visit to the Palm Beach Neurological Center, your doctor will review your medical background with special attention to present problems. A neurological exam will include tests of cranial nerves, strength, coordination, reflexes and sensation. The doctor will review any medical reports, x-rays and medications you provide. Your examination may indicate the need for additional testing to determine the diagnosis and lead to a specific treatment. A follow up visit will be scheduled to discuss the results of tests and treatment. It is very important to keep these scheduled appointments.

 

Canceling Appointments

 

A notice of two business days is required should you be unable to keep your scheduled appointment. We reserve the right to charge a fee for missed appointments.

 

Doctor-Patient Relations

 

We will make a special effort to explain your condition, medication, treatment, etc. Should you have a question or if something is not clear, please do not hesitate to ask. Our personnel, office procedures and medical equipment were chosen with much thought and care to provide the best quality medical services in a pleasant, efficient and friendly atmosphere.

 

Prescriptions and Refills

 

Have your pharmacy fax our office at (561) 694-1010, option 6 with routine requests for prescription refills during regular office hours, 9:00 a.m. – 11:45 a.m. and 1:30 p.m. – 4:30 p.m. Monday through Thursday. Every effort will be made to attend to your refill request within three (3) business days. We do however; ask that you please do not wait until you are out of medication to request a refill. Please be aware there will be times when your physician may wish to see you prior to authorizing a refill. It is our policy not to dispense or renew medications over the telephone after office hours.

 

Returning Messages

 

Our staff has been trained to answer many of your questions. When your call requires a response from your physician every effort will be made to return your call promptly, usually within 48 hours. Non-emergency phone calls should be limited to our regular business hours.

 

Evenings, Weekends & Holiday Emergencies

 

Dr. Tuchman provides clinical care in the office during regular office hours, but does not provide hospital care and is not available for emergencies on nights, weekends or holidays. We recognize that outside of office hours, medical emergencies do occur. If that happens to you, please do not delay in calling 911 for assistance or present yourself to a hospital emergency room for evaluation.

 

Fees/Billing/Insurance

 

Every effort is made to control the cost of your medical care. When scheduling your initial appointment, our office staff will obtain patient and insurance information. You will be informed of your financial responsibility prior to your appointment.

 

PAYMENT IS REQUIRED AT THE TIME SERVICES ARE RENDERED. The coded receipt given to you at the end of each visit contains the necessary information required by insurance companies to process your claim.

 

This office is a MEDICARE participating provider. If you have a Medigap policy, we will be happy to include this information on our claim to Medicare. For supplemental policies not included under Medigap, the 20% portion of the Medicare allowed fee will be collected at the time of your visit.

 

We are also participating providers with TRADITIONAL BLUE CROSS, BLUE SHIELD, UNITED HEALTHCARE, CIGNA, HUMANA, AETNA and several other insurance companies. We make every effort to verify your insurance coverage prior to your initial consultation; however, this verification is not a guarantee of payment. We recommend you contact your insurance company to verify your insurance coverage for any testing scheduled by this office at an outside facility. You are hereby advised that Palm Beach Neurological Center is NOT responsible for any charges incurred for services rendered outside of our office.

 

Any questions regarding insurance matters should be discussed with a member of our Billing Staff. We are happy to assist you in obtaining maximum benefits; however, INSURANCE CLAIMS ARE FILED AS A COURTESY, THE MEDICAL CARE PAYMENT CONTRACT IS BETWEEN YOU AND YOUR INSURANCE COMPANY; THEREFORE, THE BALANCE IS ULTIMATELY THE PATIENT’S RESPONSIBILITY.

 

For your convenience, we accept MasterCard, Visa and American Express.

 

Medical Records and Confidentiality

 

Your medical records are strictly private. Our staff will not divulge any information regarding your condition to family members, friends or your employer without your written permission. The only exception to this is when required by law, court order. A copy of your medical records will be provided upon receipt of your written request and authorization to us by mail or Fax 561-694-6921 at a nominal fee.
